1. Case Management Services

At Day Homehealth Solutions, our Case Management Services are designed to enhance patient care and optimize healthcare delivery through comprehensive coordination and support. We understand that effective case management is vital in addressing the complex needs of patients, particularly those with chronic conditions, disabilities, or those requiring multiple healthcare services

Comprehensive Chart Review

We conduct a thorough review of patient charts, including SOC OASIS, Transfer OASIS, ROC OASIS, DC OASIS, and relevant clinical notes, focusing on compliance with Medicare Conditions of Participation (COP).

Review and Feedback

We examine all clinical notes and communications, returning any deficiencies and clinical notes to the clinician along with recommendations for improvement.

Reports

We provide a Daily Census Report listing reviewed patients and identified deficiencies, along with COP Deficiency and Weekly Accomplishment Reports to support performance evaluation and clinician education and training.

Plan of Care Preparation

We prepare (or help prepare) the Plan of Care (Form 485) as directed, ensuring it meets all necessary requirements.

Direct Communication

We can assign dedicated staff, if allowed, to communicate directly with clinicians within 48 hours to address identified deficiencies.
